After subcutaneous injection, absorption is gradual, and peak plasma levels are generally reached within one to three days. Albumin binding extends the apparent half-life to roughly five days, which supports a weekly administration schedule. Metabolism proceeds mainly through proteolytic cleavage of the peptide backbone and beta-oxidation of the fatty acid chain, rather than through cytochrome P450 pathways. Eliminated fragments are largely recycled through general protein turnover, and excretion of intact drug in urine is minimal. These properties distinguish the molecule from short-acting incretin mimetics.
Tirzepatide is a synthetic peptide of 39 amino acids engineered from the native glucose-dependent insulinotropic polypeptide sequence. Its structure incorporates several non-natural residues and a C-terminal segment derived from glucagon-like peptide-1, together with a C20 fatty diacid moiety attached through a linker. The lipophilic side chain promotes binding to serum albumin, which slows renal clearance after administration. The compound is classified as a dual incretin receptor agonist and is supplied as a lyophilized powder for reconstitution or as a preformulated solution, depending on the presentation.
The peptide activates two G protein-coupled receptors, GIPR and GLP-1R. Binding triggers adenylyl cyclase activity and raises intracellular cyclic AMP in pancreatic beta cells, which potentiates insulin release when glucose is elevated. Signaling in the central nervous system is associated with reduced appetite and lower energy intake, while effects on gastric emptying and glucagon secretion are also reported. Because activity at both receptors is retained, the pharmacological profile is often described as incretin-based rather than selective for a single receptor.
Research and analytical settings increasingly require documentation of peptide origin and chain of custody. Certificate of analysis documents typically report purity by chromatographic area, mass confirmation, appearance, and residual solvent or counterion content. Independent verification by an accredited laboratory is common when a material will be used in a regulated study. Open questions remain about how well compendial methods transfer between laboratories, and about which impurity thresholds are meaningful for materials not intended for clinical use.
Peptide-based pharmaceutical products such as tirzepatide require controlled temperature management to preserve structural integrity. Manufacturer labeling generally specifies refrigeration at 2 to 8 degrees Celsius before first use, with protection from light and freezing. Exposure to repeated temperature cycling can promote aggregation or deamidation, which alters the analytical profile even when the visible solution appears unchanged. Once a product is in use, the permitted storage window and temperature range are defined by the specific labeled presentation rather than by general peptide rules.

Published work supports the view that engaging two incretin receptors produces changes in glucose handling and body weight larger than those seen with single-receptor activation. Why that difference arises is not fully settled. Open questions include how much of the observed weight effect depends on central versus peripheral signaling, and whether the two receptors form interacting complexes. Most reported findings come from controlled trials and animal models, and translation between species is imperfect. Further research is expected to refine these points over time.
Tirzepatide is a synthetic peptide built from 39 amino acid residues. Its sequence is related to human glucose-dependent insulinotropic polypeptide, with modifications that include a C-terminal extension and a C20 fatty diacid joined through a linker. Those changes raise the molecule's affinity for serum albumin, which slows renal filtration and lengthens the time it stays in circulation. The free base has an average molecular mass near 4813.5 daltons. The compound is made by solid-phase peptide synthesis followed by chromatographic purification.
At the receptor level, tirzepatide activates both the glucose-dependent insulinotropic polypeptide receptor and the glucagon-like peptide-1 receptor. Both belong to the class B family of G protein-coupled receptors and signal largely through cyclic AMP accumulation. The compound binds the two receptors with differing affinity, and the pattern of signaling at each site is described in the literature as biased rather than simply proportional to occupancy. Tissues carrying these receptors include pancreatic islets, adipose tissue, the central nervous system, and the gastrointestinal tract. The relative weight of each receptor population in producing metabolic effects continues to be studied.
==== Endogenous ==== Endogenous pyrogens are cytokines released from monocytes (which are part of the immune system). In general, they stimulate chemical responses, often in the presence of an antigen, leading to a fever. Whilst they can be a product of external factors like exogenous pyrogens, they can also be induced by internal factors like damage associated molecular patterns such as cases like rheumatoid arthritis or lupus. Major endogenous pyrogens are interleukin 1 (α and β) and interleukin 6 (IL-6). Minor endogenous pyrogens include interleukin-8, tumor necrosis factor-β, macrophage inflammatory protein-α and macrophage inflammatory protein-β as well as interferon-α, interferon-β, and interferon-γ. Tumor necrosis factor-α (TNF) also acts as a pyrogen, mediated by interleukin 1 (IL-1) release. These cytokine factors are released into general circulation, where they migrate to the brain's circumventricular organs where they are more easily absorbed than in areas protected by the blood–brain barrier. The cytokines then bind to endothelial receptors on vessel walls to receptors on microglial cells, resulting in activation of the arachidonic acid pathway. Of these, IL-1β, TNF, and IL-6 are able to raise the temperature setpoint of an organism and cause fever. These proteins produce a cyclooxygenase which induces the hypothalamic production of PGE2 which then stimulates the release of neurotransmitters such as cyclic adenosine monophosphate and increases body temperature.
A mammary gland is an exocrine gland that produces milk in mammals, including humans. Mammals get their name from the Latin word mamma, "breast". The mammary glands are arranged in organs such as the breasts in primates (for example, humans and chimpanzees), the udder in ruminants (for example, cows, goats, sheep, and deer), and the dugs of other animals (for example, dogs, cats, rabbits and pigs) to feed young offspring. Lactorrhea, the occasional production of milk by the glands, can occur in any mammal, but in most mammals, lactation, the production of enough milk for nursing, occurs only in phenotypic females who have gestated in recent months or years. It is directed by hormonal guidance from sex steroids. In a few mammalian species, male lactation can occur. With humans, male lactation can occur only under specific circumstances. Mammals are divided into 3 groups: monotremes, metatherians, and eutherians. In the case of monotremes, their mammary glands are modified sebaceous glands and without nipples. Concerning most metatherians and eutherians, only females have functional mammary glands, with the exception of some bat species. Their mammary glands can be termed as breasts or udders. In the case of breasts, each mammary gland has its own nipple (e.g., human mammary glands). In the case of udders, pairs of mammary glands comprise a single mass, with more than one nipple (or teat) hanging from it.
The supplement creatine may be helpful for well-trained athletes to increase exercise performance and strength in relation with their dietary regimen. Other popular studies done on supplements include androstenedione, chromium, and ephedra. The findings show that there are no substantial benefits from the extra intake of these supplements, yet higher health risks and costs.
The concept of emission and transmission tomography was introduced by David E. Kuhl, Luke Chapman and Roy Edwards in the late 1950s. Their work would lead to the design and construction of several tomographic instruments at Washington University School of Medicine and later at the University of Pennsylvania. In the 1960s and 70s tomographic imaging instruments and techniques were further developed by Michel Ter-Pogossian, Michael E. Phelps, Edward J. Hoffman and others at Washington University School of Medicine. Work by Gordon Brownell, Charles Burnham and their associates at the Massachusetts General Hospital beginning in the 1950s contributed significantly to the development of PET technology and included the first demonstration of annihilation radiation for medical imaging. Their innovations, including the use of light pipes and volumetric analysis, have been important in the deployment of PET imaging. In 1961, James Robertson and his associates at Brookhaven National Laboratory built the first single-plane PET scan, nicknamed the "head-shrinker". One of the factors most responsible for the acceptance of positron imaging was the development of radiopharmaceuticals. In particular, the development of labeled 2-fluorodeoxy-D-glucose (FDG—firstly synthethized and described by two Czech scientists from Charles University in Prague in 1968) by the Brookhaven group under the direction of Al Wolf and Joanna Fowler was a major factor in expanding the scope of PET imaging.

=== Use in exercise and sports === Pseudoephedrine has been used as a performance-enhancing drug in exercise and sports due to its sympathomimetic and stimulant effects. Because of these effects, pseudoephedrine can increase heart rate, elevate blood pressure, improve mental energy, and reduce fatigue, among other performance-enhancing effects. A 2015 systematic review found that pseudoephedrine lacked performance-enhancing effects at therapeutic doses (60–120 mg) but significantly enhanced athletic performance at supratherapeutic doses (≥180 mg). A subsequent 2018 meta-analysis, which included seven additional studies, found that pseudoephedrine had a small positive effect on heart rate (SMDTooltip standardized mean difference = 0.43) but insignificant effects on time trials, perceived exertion ratings, blood glucose levels, and blood lactate levels. However, subgroup analyses revealed that effect sizes were larger for heart rate increases and quicker time trials in well-trained athletes and younger participants, for shorter exercise sessions with pseudoephedrine administered within 90 minutes beforehand, and with higher doses of pseudoephedrine. A dose–response relationship was established, with larger doses (>170 mg) showing greater increases in heart rate and faster time trials than with smaller doses (≤170 mg) (SMD = 0.85 for heart rate and SMD = -0.24 for time trials, respectively). In any case, the meta-analysis concluded that the performance-enhancing effects of pseudoephedrine were marginal to small and likely to be lower in magnitude than with caffeine.
=== Occurrence in humans and the biosphere === Polonium-210 is widespread in the biosphere, including in human tissues, because of its position in the uranium-238 decay chain. Natural uranium-238 in the Earth's crust decays through a series of solid radioactive intermediates including radium-226 to the radioactive noble gas radon-222, some of which, during its 3.8-day half-life, diffuses into the atmosphere. There it decays through several more steps to polonium-210, much of which, during its 138-day half-life, is washed back down to the Earth's surface, thus entering the biosphere, before finally decaying to stable lead-206. As early as the 1920s, French biologist Antoine Lacassagne, using polonium provided by his colleague Marie Curie, showed that the element has a specific pattern of uptake in rabbit tissues, with high concentrations, particularly in liver, kidney, and testes. More recent evidence suggests that this behavior results from polonium substituting for its congener sulfur, also in group 16 of the periodic table, in sulfur-containing amino-acids or related molecules and that similar patterns of distribution occur in human tissues. Polonium is indeed an element naturally present in all humans, contributing appreciably to natural background dose, with wide geographical and cultural variations, and particularly high levels in arctic residents, for example.
This concern is not the same as the serotonin-toxicity risk seen when MAOIs are combined with drugs that significantly inhibit serotonin reuptake or release serotonin. A classic MAOI-to-classic MAOI overlap is not expected to cause serotonin toxicity unless another serotonergic drug with clinically significant serotonin reuptake inhibition or serotonin-releasing activity is also involved. This should be distinguished from medically supervised switching between classic MAOIs, where shorter washouts, direct switches, or cautious cross-tapering have been used by some expert clinicians under close monitoring. In that context, temporary MAOI-to-MAOI overlap may have practical utility, such as reducing relapse risk or avoiding a prolonged untreated interval when changing from one MAOI to another because of adverse effects or inadequate response. Drugs described as "stimulants" are not a uniform pharmacological category and are not uniformly contraindicated with phenelzine. Modern MAOI guidance distinguishes amphetamines by dose: medium or high doses are avoided because of serotonin-releasing activity, while low-dose amphetamines are listed as a strong relative contraindication rather than an absolute contraindication. When used by experienced clinicians, they require specialist caution, reduced starting doses, slow titration, and blood-pressure monitoring. Lisdexamfetamine may be lower risk than some other amphetamine preparations because of its lower peak plasma concentration and longer time to peak concentration.

In December 1951, Solomon's son Cyprian Bhekuzulu kaSolomon was officially recognised as the Paramount Chief of the Zulu people, but real power over ordinary Zulu people lay with South African government officials working through local chiefs who could be removed from office for failure to cooperate.

== Deficiency == Complement defects are associated with an increased risk of infectious or local and inflammatory thrombotic disorders. These complement-linked disorders are rare but tend to show up during childhood. Hereditary angioedema (HAE) result from impaired function of the C1 inhibitor, and complement disorders result in renal disorders, including atypical hemolytic uremic syndrome (aHUS) or C3 glomerulopathy (C3G). Properdin deficiency is a rare X-linked disease in which properdin is deficient. Affected individuals are susceptible to fulminant meningococcal disease, whereas defects of the classical pathway increase the risk of autoimmune disorders. Properdin deficiency has been reported in more than 70 patients, and is linked to infections with Neisseria meningitides and Neisseria gonorrhoea. Mortality rates are higher in individuals with properdin deficiency in comparison to those with terminal complement deficiencies. Three classes of properdin deficiencies are
Measures to remove potassium from the body include diuretics such as furosemide, potassium-binders such as polystyrene sulfonate (Kayexalate) and sodium zirconium cyclosilicate, and hemodialysis. Hemodialysis is the most effective method. Hyperkalemia is rare among those who are otherwise healthy. Among those who are hospitalized, rates are between 1% and 2.5%. It is associated with an increased mortality, whether due to hyperkalaemia itself or as a marker of severe illness, especially in those without chronic kidney disease. The word hyperkalemia comes from hyper- 'high' + kalium 'potassium' + -emia 'blood condition'.
Typically, people have fewer than 36 repeats in the polyQ region, which results in the normal production and cytoplasmic localization of the huntingtin. However, 36 or more repeats result in the production of a mutant huntingtin (mHTT), which causes degeneration of medium spiny neurons. Regions of the brain have differing amounts and reliance on these types of neurons and are affected accordingly. Generally, the number of CAG repeats is related to how much this process is affected, and accounts for about 60% of the variation of the age of the onset of symptoms. The remaining variation is attributed to the environment and other genes that modify the mechanism of HD. About 36 to 39 repeats result in a reduced-penetrance form of the disease, with a much later onset and slower progression of symptoms. In some cases, the onset may be so late that symptoms are never noticed. With very large repeat counts (more than 60), HD onset can occur below the age of 20, known as juvenile HD. Juvenile HD is typically of the Westphal variant that is characterized by slowness of movement, rigidity, and tremors. This accounts for about 7% of HD carriers.
The administration of President Álvaro Uribe (2002–2010) adopted the democratic security policy which included an integrated counter-terrorism and counter-insurgency campaign. The government economic plan also promoted confidence in investors. As part of a controversial peace process, the AUC (right-wing paramilitaries) had ceased to function formally as an organization . In February 2008, millions of Colombians demonstrated against FARC and other outlawed groups. After peace negotiations in Cuba, the Colombian government of President Juan Manuel Santos and the guerrillas of the FARC-EP announced a final agreement to end the conflict. However, a referendum to ratify the deal was unsuccessful. Afterward, the Colombian government and the FARC signed a revised peace deal in November 2016, which the Colombian congress approved. In 2016, President Santos was awarded the Nobel Peace Prize. The Government began a process of attention and comprehensive reparation for victims of conflict. Colombia shows modest progress in the struggle to defend human rights, as expressed by HRW. A Special Jurisdiction of Peace has been created to investigate, clarify, prosecute and punish serious human rights violations and grave breaches of international humanitarian law which occurred during the armed conflict and to satisfy victims' right to justice. During his visit to Colombia, Pope Francis paid tribute to the victims of the conflict.

It is a synthetic linear peptide that acts as a dual agonist at the GIP and GLP-1 receptors. It combines a modified incretin backbone with a fatty diacid side chain that extends its circulation time. It is not a small-molecule drug and is not orally absorbed in its native form.
Single-receptor agents act predominantly at GLP-1R. Tirzepatide retains activity at GIPR, which is expressed in pancreatic islets, adipose tissue, and the central nervous system. The contribution of each receptor to the overall response is still an area of active study, and the relative weight of GIPR signaling in humans is not fully settled.
The fatty diacid moiety binds serum albumin, which reduces renal filtration of the active peptide. Additional resistance to enzymatic breakdown comes from non-natural residues in the backbone. Together these features produce an apparent half-life of about five days and permit weekly dosing intervals.
The peptide backbone and its fatty acid side chain are susceptible to degradation at elevated temperatures. Refrigeration slows hydrolysis, oxidation, and aggregation processes. Labeled storage ranges reflect stability data generated under defined conditions.
