Tirzepatide is a synthetic peptide of 39 amino acids that carries a C20 fatty diacid side chain attached through a linker. Its molecular formula is C225H348N48O68, and its molecular weight is about 4813 daltons. The compound belongs to the incretin mimetic class and is administered by subcutaneous injection. The fatty acid chain promotes binding to serum albumin, which slows renal clearance and extends the circulation time of the molecule. It was identified during screening of sequences derived from glucose-dependent insulinotropic polypeptide.
Tirzepatide activates both the glucose-dependent insulinotropic polypeptide receptor and the glucagon-like peptide-1 receptor, making it a dual agonist rather than a selective agent. Engagement of the GLP-1 receptor is linked to glucose-dependent insulin release, slower gastric emptying, and reduced appetite signalling. The relative contribution of the GIP arm remains an active research question; proposed roles include improved insulin sensitivity and altered adipose tissue handling. Receptor occupancy studies suggest the molecule interacts with both targets at circulating concentrations achieved during therapy.
Development began in the 2010s, when researchers modified a GIP-based scaffold to add GLP-1 activity and then attached the fatty diacid to lengthen its half-life. Clinical evaluation proceeded through large phase 3 programmes in type 2 diabetes and in obesity, and regulators in the United States cleared the compound for type 2 diabetes in 2022 and for chronic weight management in 2023. Several cardiovascular and metabolic outcome studies are still reporting, so the picture of long-term benefit and risk is incomplete. Approvals in other regions followed on different timelines.
Pharmacologically, tirzepatide activates two distinct G protein-coupled receptors: the glucose-dependent insulinotropic polypeptide receptor and the glucagon-like peptide-1 receptor. Binding at each target triggers cyclic AMP accumulation and downstream signaling in pancreatic beta cells, adipose tissue and the central nervous system. Because the two pathways overlap only partially, the combined effect on insulin secretion, glucagon suppression and appetite signaling differs from that of selective single-receptor compounds. Affinity is not equal across the two targets, and the clinical meaning of that imbalance remains an area of active study.
Clinical research programs have evaluated tirzepatide in adults with type 2 diabetes and in adults with obesity or excess weight. Trials generally reported reductions in glycated hemoglobin and body weight across treatment periods of several months. Since these studies enrolled defined populations under controlled conditions, the findings describe group averages rather than individual outcomes. Open questions include the durability of effects after treatment stops, variation among subgroups, and the long-term consequences of sustained dual receptor stimulation. Published trial summaries should be consulted for exact measurements rather than secondary accounts.

Identity and purity are assessed by reversed-phase high-performance liquid chromatography, with mass confirmation by electrospray ionisation mass spectrometry. Peptide mapping after enzymatic digestion verifies the primary sequence. Size-exclusion chromatography quantifies aggregates, while circular dichroism provides a secondary-structure fingerprint. Bioanalytical quantification in plasma uses immunoassay or LC-MS/MS. Reported purity for research-grade lots is commonly 95 percent or higher, and residual water content is checked by Karl Fischer titration.
As a peptide, tirzepatide is handled as a lyophilised solid in research settings and as a preserved solution in finished products. Aqueous solubility is pH dependent and reaches a minimum near the isoelectric point, which lies close to pH 5.4. Stock solutions are typically prepared in neutral or slightly basic buffer to limit precipitation. The solid is hygroscopic and should be equilibrated to room temperature before opening so that condensation does not form on the powder surface.
Recommended storage for reference material is a freezer at approximately -20 degrees Celsius, protected from light and moisture. Commercial injectable presentations are stored refrigerated between 2 and 8 degrees Celsius and must not be frozen. Product labelling generally permits a limited period at controlled room temperature once dispensed, with the exact window depending on the presentation. Repeated temperature cycling is avoided because it can promote aggregation or deamidation of the peptide chain.
Dual agonism at the GIP and GLP-1 receptors underlies the observed pharmacology. Activation of GLP-1 receptors raises glucose-dependent insulin release, lowers glucagon secretion, slows gastric emptying and reduces appetite. GIP receptor activation contributes additional effects on adipose tissue and on energy balance, and the combined action on appetite appears larger than either pathway alone in animal models. Signalling bias and the relative contribution of each receptor arm to weight-related effects remain areas of active investigation.
Structure-activity work shows that fatty acid length, linker chemistry and the position of acylation all influence albumin affinity and receptor potency. Plasma protein binding exceeds 99 percent, which restricts distribution and slows renal clearance. Degradation proceeds largely through general proteolysis and fatty acid oxidation rather than cytochrome P450 metabolism, so exposure to common oxidative drug interactions is limited. Whether these clearance routes vary meaningfully between individuals is not fully established.
in situ hybridization (ISH) A hybridization probe assay in which a labeled, single-stranded DNA or RNA molecule or nucleic acid analogue containing a sequence that is complementary to a particular DNA or RNA sequence is allowed to hybridize with its complement in situ, i.e. in its natural context, such as within cells or tissue sections (as opposed to within homogeneous samples extracted from cells or tissues, where cellular or histological structure has been lost in the process of obtaining the sample), in order to reveal the precise location of the complementary sequence within this context. The label may be a radioactive compound, fluorescent molecule, or hapten, permitting detection by a variety of visualization techniques. In situ hybridization is commonly used to identify the physical locations of specific DNA sequences such as genes and regulatory elements on chromosomes, which can provide insight into chromosomal structure and integrity; to determine the subcellular locations where various types of RNA accumulate and interact with other molecules; and to visualize the tissues and organs within an organism where specific genes are expressed at various developmental stages (by probing for the genes' RNA transcripts).
American diplomat Henry Kissinger (1923–2023) played an important and controversial role in the Vietnam War. Starting out as a supporter, Kissinger came to see it as a drag on American power. In 1968, Kissinger leaked information about the status of the peace talks in Paris to the Nixon campaign and was rewarded with being appointed National Security Advisor under Richard Nixon. As National Security Advisor, Kissinger sought initially to find a way to end the war on American terms. During his tenure, Kissinger came to differ with Nixon as Kissinger was more in favor of seeking an end to war as expeditiously as possible with minimum damage to American prestige. In October 1972, Kissinger reached a draft agreement that Nixon at first rejected, leading to the Christmas bombings of December 1972. The agreement that Kissinger signed in January 1973—which led to the American withdrawal from Vietnam in March of that year—was very similar to the draft agreement rejected the previous year. As National Security Advisor and Secretary of State, Kissinger favored continued American support for South Vietnam right until the collapse of that state in April 1975, which Kissinger blamed on Congress.
== Function == Originating from the embryonic epidermis, the hair follicle evolves into one of the most complex structures in the human body, comprising 7–8 distinct tissue sections. The base of the hair follicle contains the bulb, housing dermal fibroblasts known as the dermal papilla, crucial for morphogenesis and the hair follicle's cyclic activity. Encircling these cells is the matrix cell region, the hair follicle's proliferative compartment, responsible for the formation of different follicle compartments (except the ORS) and the production of crucial structural elements of hair - hair keratins and associated proteins known as KAPs. Keratin is a crucial fibrous protein found in animals, constituting tough structures like hair, feathers, nails, and horns. It's classified based on tissue origin and sulfur content: soft keratins have lower sulfur, while hard keratins, found in hair and claws, contain more sulfur, creating a stronger structure. Keratins belong to two types - acidic Type I and neutral-basic Type II, further categorized into Type I a and b, and Type II a and b. The initial step in forming keratin is the alignment of type I and type II keratin polypeptides to create a heterodimer, which then aggregates into higher-order structural units. Similar to other intermediate filament subunit proteins, a prevalent secondary structure exists: a well-preserved, central alpha-helical domain made up of four coiled-coil segments along with non-helical end-terminal domains that vary in sequences and lengths [14].
Shriram Gadhve of All India Vegetable Growers Association (AIVGA) claims his organisation supports retail reform. He claimed that currently, it is the middlemen commission agents who benefit at the cost of farmers. He urged that the retail reform must focus on rural areas and that farmers receive benefits. Gadhve claimed, "A better cold storage would help since this could help prevent the existing loss of 34% of fruits and vegetables due to inefficient systems in place." AIVGA operates in nine states including Maharashtra, Andhra Pradesh, West Bengal, Bihar, Chhattisgarh, Punjab and Haryana with 2,200 farmer outfits as its members. Bharat Krishak Samaj, a farmer association with more than 75,000 members says it supports retail reform. Ajay Vir Jakhar, the chairman of Bharat Krishak Samaj, claimed a monopoly exists between the private guilds of middlemen, commission agents at the sabzi mandis (India's wholesale markets for vegetables and farm produce) and the small shopkeepers in the unorganised retail market. Given the perishable nature of food like fruit and vegetables, without the option of safe and reliable cold storage, the farmer is compelled to sell his crop at whatever price he can get. He cannot wait for a better price and is thus exploited by the current monopoly of middlemen. Jakhar asked that the government make it mandatory for organised retailers to buy 75% of their produce directly from farmers, bypassing the middlemen monopoly and India's sabzi mandi auction system. Consortium of Indian Farmers Associations (CIFA) announced its support for retail reform.

=== Imaging === The role of MRI or ultrasound imaging in the diagnosis of CTS is unclear. Their routine use is not recommended. Morphological MRI has high sensitivity but low specificity for CTS. High signal intensity may suggest accumulation of axonal transport, myelin sheath degeneration, or edema. However, more recent quantitative MRI techniques, which derive repeatable, reliable, and objective biomarkers from nerves and skeletal muscle, may have utility, including diffusion-weighted (typically diffusion tensor) MRI, which has demonstrable normal values and aberrations in carpal tunnel syndrome.

In addition, naloxone could precipitate withdrawal symptoms in SR-17018-treated mice, though these withdrawal symptoms were less intense than with other MOR agonists. Chronic morphine administration causes cross-tolerance to analgesia with SR-17018 in mice and to the same magnitude as with morphine. This suggests that SR-17018 is accessing the same pool of MORs as morphine to produce its analgesic effects. Conversely, chronic SR-17018 administration did not produce cross-tolerance with morphine, which retained full analgesic sensitivity. This is consistent with SR-17018's lack of MOR desensitization. Surprisingly, chronic administration of morphine followed by replacement with chronic SR-17018 administration resulted in restoration of SR-17018's analgesic efficacy and full restoration of morphine's analgesic potency within a few days. Conversely, replacement with buprenorphine instead did not restore morphine's analgesic potency. These findings indicate that SR-17018 substitution can reverse morphine analgesic tolerance, whereas substitution with classical opioids like buprenorphine is unable to do this. The mechanism underlying this effect is unknown. Aside from effects on analgesic tolerance, both SR-17018 and buprenorphine suppressed morphine withdrawal symptoms. Similarly to SR-17018, the MOR positive allosteric modulator BMS-986122 has also been found to attenuate opioid analgesic tolerance in rodents.
== History == The effectiveness of velmanase alfa was evaluated in participants with alpha-mannosidosis in a phase III multicenter, randomized, double-blind, placebo-controlled, parallel group study. The trial evaluated the efficacy of velmanase alfa over 52 weeks at a dose of 1 mg/kg given weekly as an intravenous infusion. A total of 25 participants were enrolled (14 males, 11 females), including 13 adult participants (age range: ≥18 to 35 years; mean: 25 years) and 12 pediatric participants (age range: ≥6 to <18 years; mean: 11 years); all participants were White. Fifteen participants (8 adult and 7 pediatric) received velmanase alfa and 10 participants (5 adult and 5 pediatric) received placebo.
Similarly, a stereogenic axis (or plane) is defined as an axis (or plane) in the molecule such that the swapping of any two ligands attached to the axis (or plane) gives rise to a stereoisomer. For instance, the C2-symmetric species 1,1′-bi-2-naphthol (BINOL) and 1,3-dichloroallene have stereogenic axes and exhibit axial chirality, while (E)-cyclooctene and many ferrocene derivatives bearing two or more substituents have stereogenic planes and exhibit planar chirality. Chirality can also arise from isotopic differences between substituents, such as in the deuterated benzyl alcohol PhCHDOH; which is chiral and optically active ([α]D = 0.715°), even though the non-deuterated compound PhCH2OH is not. If two enantiomers easily interconvert, the pure enantiomers may be practically impossible to separate, and only the racemic mixture is observable. This is the case, for example, of most amines with three different substituents (NRR′R″), because of the low energy barrier for nitrogen inversion. When the optical rotation for an enantiomer is too low for practical measurement, the species is said to exhibit cryptochirality. Chirality is an intrinsic part of the identity of a molecule, so the systematic name includes details of the absolute configuration (R/S, D/L, or other designations).

The dioxide is soluble in acidic solutions (the monoxide GeO, is even more so), and this is sometimes used to classify germanium as a metal. Up to the 1930s germanium was considered to be a poorly conducting metal; it has occasionally been classified as a metal by later writers. As with all the elements commonly recognised as metalloids, germanium has an established organometallic chemistry (see Organogermanium chemistry).
On 18 August 2015, following a 112-point loss to Adelaide in Round 20, 2015—and a dismal season overall for the Essendon Football Club, by which time they had a 5–14 win–loss record and were fifteenth on the AFL ladder—Hird resigned as senior coach.
In order to overcome the insufficient prediction of binding affinity calculated by recent scoring functions, the protein-ligand interaction and compound 3D structure information are used for analysis. For structure-based drug design, several post-screening analyses focusing on protein-ligand interaction have been developed for improving enrichment and effectively mining potential candidates:
While not meeting the previous definition of (transcriptional) attenuation, these are now considered to be variants of the same phenomena and are included in this article. Attenuation is an ancient regulatory system, prevalent in many bacterial species providing fast and sensitive regulation of gene operons and is commonly used to repress genes in the presence of their own product (or a downstream metabolite).

It is a synthetic 39-amino-acid peptide that acts on two incretin receptors, the GIP receptor and the GLP-1 receptor. It is given by subcutaneous injection and has a circulating half-life of roughly five days. It is not a small molecule and is not absorbed usefully from the gut in conventional oral form.
Selective agents act on one receptor, while tirzepatide engages both GIP and GLP-1 receptors. The added GIP activity may contribute effects on insulin sensitivity and on fat metabolism. Whether the dual action produces meaningful clinical advantages beyond differences in potency is still being examined.
The downstream consequences of GIP receptor activation are not fully characterised in humans. It is also unclear how much each receptor contributes to appetite reduction and to shifts in body composition. Published work describes associations and proposed pathways rather than settled causal chains.
It is a synthetic peptide that activates both the GIP and GLP-1 receptors, making it a dual agonist. Approved products are given by injection rather than by mouth. It is not a small molecule and does not belong to the older sulfonylurea or thiazolidinedione families.
