Clinical development proceeded through large phase 3 programmes in type 2 diabetes and in obesity or overweight with at least one weight-related comorbidity. Regulatory approvals followed in several jurisdictions for both indications. Weekly subcutaneous dosing reflects an elimination half-life of roughly five days. Open questions include the durability of metabolic effects after treatment stops, long-term cardiovascular and hepatic outcomes beyond completed trials, and whether the dual mechanism confers benefits independent of total receptor occupancy. Published literature continues to expand on these points. Substantial uncertainty remains about interindividual variability in response.
Tirzepatide is a synthetic linear peptide of 39 amino acids that acts as a dual agonist at the glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptors. Its sequence derives from native GIP but incorporates non-natural residues and a C20 fatty diacid moiety linked to a lysine side chain. The lipophilic chain promotes albumin binding, which slows renal clearance and extends circulation time. The unmodified peptide has a molecular formula of C225H348N48O68 and a molecular mass near 4,813 daltons.
Receptor activation by tirzepatide raises intracellular cyclic AMP through Gs-coupled signalling at both targets. At the GLP-1 receptor the downstream effect includes glucose-dependent insulin release, suppressed glucagon secretion, delayed gastric emptying, and reduced appetite signalling in the hypothalamus. GIP receptor engagement adds insulinotropic activity and appears to influence lipid handling in adipose tissue. Because both receptors are stimulated at the same time, the pharmacological profile differs from that of selective GLP-1 receptor agonists, and the relative contribution of each arm remains an area of active investigation.
An extended fatty diacid moiety promotes binding to serum albumin, which slows renal clearance and extends the circulating half-life to roughly five days. That property supports once-weekly administration and largely explains the dosing interval described in clinical reports. Published data come mainly from large randomised programmes that evaluated glycaemic control and body weight over periods of many months. Long-term outcomes beyond those trial windows, including what happens after treatment stops, remain an active area of investigation.
Tirzepatide is a synthetic peptide built from thirty-nine amino acids. Its sequence is derived from native glucose-dependent insulinotropic polypeptide, or GIP, with several non-natural residues and a fatty diacid side chain attached through a linker. The molecule behaves as a dual agonist at two incretin receptors, GIP and GLP-1, instead of targeting a single receptor. This dual engagement separates it from earlier single-receptor incretin compounds and underpins most of its reported pharmacological activity.

In clinical research, tirzepatide has been studied in randomized controlled trials for glycemic control and body weight reduction. These trials typically measure changes in hemoglobin A1c and body weight over periods of several months. The drug is administered by subcutaneous injection, and its pharmacokinetic profile supports once-weekly dosing. Post-marketing surveillance continues to evaluate long-term outcomes and rare adverse events.
Tirzepatide is a synthetic peptide that acts as a dual agonist at the glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptors. The molecule contains 39 amino acids and features a C20 fatty diacid moiety attached via a linker, which promotes albumin binding and extends its circulating half-life. Its sequence incorporates non-natural amino acids and modifications that reduce susceptibility to degradation by dipeptidyl peptidase-4. This dual receptor activity distinguishes it from selective GLP-1 receptor agonists.
The GIP receptor is expressed in pancreatic islets, adipose tissue, and the central nervous system, while GLP-1 receptors are found in pancreatic islets, the gastrointestinal tract, and the brain. Activation of both receptors can enhance glucose-dependent insulin secretion and reduce glucagon release. The relative contribution of each receptor to the overall pharmacological effect remains an area of ongoing investigation. Preclinical studies suggest that GIP receptor agonism may modulate appetite and energy balance, but the precise mechanisms in humans are not fully established.
Tirzepatide activates both the glucose-dependent insulinotropic polypeptide receptor and the glucagon-like peptide-1 receptor, making it a dual agonist rather than a selective agent. Engagement of the GLP-1 receptor is linked to glucose-dependent insulin release, slower gastric emptying, and reduced appetite signalling. The relative contribution of the GIP arm remains an active research question; proposed roles include improved insulin sensitivity and altered adipose tissue handling. Receptor occupancy studies suggest the molecule interacts with both targets at circulating concentrations achieved during therapy.
Development began in the 2010s, when researchers modified a GIP-based scaffold to add GLP-1 activity and then attached the fatty diacid to lengthen its half-life. Clinical evaluation proceeded through large phase 3 programmes in type 2 diabetes and in obesity, and regulators in the United States cleared the compound for type 2 diabetes in 2022 and for chronic weight management in 2023. Several cardiovascular and metabolic outcome studies are still reporting, so the picture of long-term benefit and risk is incomplete. Approvals in other regions followed on different timelines.
Tirzepatide is a synthetic peptide of 39 amino acids that carries a C20 fatty diacid side chain attached through a linker. Its molecular formula is C225H348N48O68, and its molecular weight is about 4813 daltons. The compound belongs to the incretin mimetic class and is administered by subcutaneous injection. The fatty acid chain promotes binding to serum albumin, which slows renal clearance and extends the circulation time of the molecule. It was identified during screening of sequences derived from glucose-dependent insulinotropic polypeptide.
Fuel savings: According to the GITI, for every 10% of under-inflation on each tire on a vehicle, a 1% reduction in fuel economy will occur. In the United States alone, the Department of Transportation estimates that under inflated tires waste 2 billion US gallons (7,600,000 m3) of fuel each year. Extended tire life: Under inflated tires are the number one cause of tire failure and contribute to tire disintegration, heat buildup, ply separation and sidewall/casing breakdowns. Further, a difference of 10 pounds per square inch (69 kPa; 0.69 bar) in pressure on a set of duals literally drags the lower pressured tire 2.5 metres per kilometre (13 feet per mile). Moreover, running a tire even briefly on inadequate pressure breaks down the casing and prevents the ability to retread. Not all sudden tire failures are caused by under-inflation. Structural damages caused, for example, by hitting sharp curbs or potholes, can also lead to sudden tire failures, even a certain time after the damaging incident. These cannot be proactively detected by any TPMS. Improved safety: Under-inflated tires lead to tread separation and tire failure, resulting in 40,000 accidents, 33,000 injuries and over 650 deaths per year. Properly inflated tires provide greater stability, handling, and braking efficiency. Environmental efficiency: Under-inflated tires, as estimated by the US Department of Transportation, release over 26 billion kilograms (57.5 billion pounds) of unnecessary carbon-monoxide pollutants into the atmosphere each year in the United States alone.
A partial vacuum may be generated by increasing the volume of a container. To continue evacuating a chamber indefinitely without requiring infinite growth, a compartment of the vacuum can be repeatedly closed off, exhausted, and expanded again. This is the principle behind a positive displacement pump, for example the manual water pump. Inside the pump, a mechanism expands a small sealed cavity to reduce its pressure below that of the atmosphere. Because of the pressure differential, some fluid from the chamber (or the well, in our example) is pushed into the pump's small cavity. The pump's cavity is then sealed from the chamber, opened to the atmosphere, and squeezed back to a minute size. More sophisticated systems are used for most industrial applications, but the basic principle of cyclic volume removal is the same:
For those with neuroendocrine cancers the number alive after five years is much better at 65%, varying considerably with type. A solid pseudopapillary tumour is a low-grade malignant tumour of the pancreas of papillary architecture that typically afflicts young women.
=== Biosynthesis === Like other steroid hormones, testosterone is derived from cholesterol (Figure 1). The first step in the biosynthesis involves the oxidative cleavage of the side-chain of cholesterol by cholesterol side-chain cleavage enzyme (P450scc, CYP11A1), a mitochondrial cytochrome P450 oxidase with the loss of six carbon atoms to give pregnenolone. In the next step, two additional carbon atoms are removed by the CYP17A1 (17α-hydroxylase/17,20-lyase) enzyme in the endoplasmic reticulum to yield a variety of C19 steroids. In addition, the 3β-hydroxyl group is oxidized by 3β-hydroxysteroid dehydrogenase to produce androstenedione. In the final and rate limiting step, the C17 keto group androstenedione is reduced by 17β-hydroxysteroid dehydrogenase to yield testosterone. The largest amounts of testosterone (>95%) are produced by the testes in men, while the adrenal glands account for most of the remainder. Testosterone is also synthesized in far smaller total quantities in women by the adrenal glands, thecal cells of the ovaries, and, during pregnancy, by the placenta. In the testes, testosterone is produced by the Leydig cells. The male generative glands also contain Sertoli cells, which require testosterone for spermatogenesis. Like most hormones, testosterone is supplied to target tissues in the blood where much of it is transported bound to a specific plasma protein, sex hormone-binding globulin (SHBG).
=== Pacific strikes === The US struck another alleged drug boat on 21 October, killing at least two people, marking the first strike to take place in the Pacific Ocean. An unnamed US official said the strike occurred off the Colombian coast. A second strike in the Pacific on 22 October killed three people. Three more strikes on four alleged drug boats in the Pacific—the first instance of multiple boats struck in one day—killed 14 people on 27 October. There was one survivor, seen clinging to debris after the strike. The Mexican Navy (SEMAR) coordinated search and rescue after the US reported the location to aircraft in the area from Mexico, and the US Coast Guard. On 31 October, Mexican officials said that after its mandatory, typical 96-hour search had yielded no survivor, it would stop searching. On 29 October, a strike on an alleged drug boat in the eastern Pacific Ocean killed four people, followed by a 4 November strike that killed two people. Two boats were struck on 9 November, each killing three people. In the first strike after the unveiling of Operation Southern Spear, the US Southern Command announced in a post to X that the Southern Spear joint task force had conducted a 15 November strike that killed three people; a strike on 4 December killed four people, and three strikes on three vessels that killed eight people were announced on 15 December. A strike on 17 December killed four people, two strikes on two vessels on 18 December killed five people, a 22 December strike killed one person, and a 29 December strike killed two people.

=== Adverse effects === In humans, exposure to α-bungarotoxin can lead to various symptoms, such as headache, dizziness, unconsciousness, visual and speech disturbances, and occasionally seizures. Onset of severe abdominal pain and muscular paralysis within 10 hours and may last for 4 days. Finally, respiratory paralysis can lead to death. Additionally, it can also lead to mild symptoms like dermatitis and allergic reactions, or stronger symptoms like blood coagulation, disseminated intravascular coagulation, tissue injury, and hemorrhage. In animals, studies have been done to analyze the effect of the α-bungarotoxin on animals. One study showed this toxin causing paralysis in chickens by blocking neuromuscular transmission at the motor end-plate. This led to muscle weakness and ultimately, paralysis. In ancient days, these venoms were already widespread across the world. Then, folklore medicine utilized plant-based and bioactive inhibitor compounds to treat bites from venomous animals like snakes and scorpions. This approach proved successful in preventing envenomation, effectively mitigating the harmful effects of venom on the victims. Today, treatment for krait bites involves antivenom, which can lead to various undesirable and potentially life-threatening side effects, such as nausea, urticarial, hypotension, cyanosis, and severe allergic reactions.
The term narcotic (, from ancient Greek ναρκῶ narkō, "I make numb") originally referred medically to any psychoactive compound with numbing or paralyzing properties. In the United States, it has since become associated with opiates and opioids, commonly morphine and heroin, as well as derivatives of many of the compounds found within raw opium latex. Legally speaking, the term "narcotic" may be imprecisely defined and typically has negative connotations. When used in a legal context in the U.S., a narcotic drug is totally prohibited, such as heroin, or one that is used in violation of legal regulation (in this word sense, equal to any controlled substance or illicit drug). In the medical community, the term is more precisely defined as a synonym of opioids and generally does not carry the same negative connotations. Statutory classification of a drug as a narcotic often increases the penalties for violation of drug control statutes. For example, although U.S. federal law classifies both cocaine and amphetamines as "Schedule II" drugs, the penalty for possession of cocaine is greater than the penalty for possession of amphetamines because cocaine, unlike amphetamines, is classified as a narcotic. Research acknowledges that alcohol can have similar effects to narcotics in head or trunk trauma situations.
=== Other uses === Topical analgesia is generally recommended to avoid systemic side-effects. Painful joints, for example, may be treated with an ibuprofen- or diclofenac-containing gel (The labeling for topical diclofenac has been updated to warn about drug-induced hepatotoxicity.); capsaicin also is used topically. Lidocaine, an anesthetic, and steroids may be injected into joints for longer-term pain relief. Lidocaine is also used for painful mouth sores and to numb areas for dental work and minor medical procedures. In February 2007 the FDA notified consumers and healthcare professionals of the potential hazards of topical anesthetics entering the bloodstream when applied in large doses to the skin without medical supervision. These topical anesthetics contain anesthetic drugs such as lidocaine, tetracaine, benzocaine, and prilocaine in a cream, ointment, or gel.
=== Discovery === Copernicium was first created on 9 February 1996, at the Gesellschaft für Schwerionenforschung (GSI) in Darmstadt, Germany, by Sigurd Hofmann, Victor Ninov et al. This element was created by firing accelerated zinc-70 nuclei at a target made of lead-208 nuclei in a heavy ion accelerator. A single atom of copernicium was produced with a mass number of 277. (A second was originally reported, but was found to have been based on data fabricated by Ninov, and was thus retracted.)
Brown had high blood pressure, a developmental delay, attention deficit hyperactivity disorder (ADHD), and a chromosomal disorder that caused a mild intellectual disability and blurry vision, according to a wrongful death lawsuit filed by family members. Social media commentators began to nickname the drink "the lemonade that kills you". The caffeine content was later reduced by Panera in December 2023, with the 30-ounce drink reduced to 237 mg and the 20-ounce to 158 mg. In May 2024, the company announced they would begin phasing out the drink.

== Toxicodynamics == Apamin is the smallest neurotoxin polypeptide known, and the only one that passes the blood-brain barrier. Apamin thus reaches its target organ, the central nervous system. Here it inhibits small-conductance Ca2+-activated K+ channels (SK channels) in neurons. These channels are responsible for the afterhyperpolarizations that follow action potentials, and therefore regulate the repetitive firing frequency. Three different types of SK channels show different characteristics. Only SK2 and SK3 are blocked by apamin, whereas SK1 is apamin insensitive. SK channels function as a tetramer of subunits. Heteromers have intermediate sensitivity. SK channels are activated by the binding of intracellular Ca2+ to the protein calmodulin, which is constitutively associated to the channel. Transport of potassium ions out of the cell along their concentration gradient causes the membrane potential to become more negative. The SK channels are present in a wide range of excitable and non-excitable cells, including cells in the central nervous system, intestinal myocytes, endothelial cells, and hepatocytes. Binding of apamin to SK channels is mediated by amino acids in the pore region as well as extracellular amino acids of the SK channel. It is likely that the inhibition of SK channels is caused by blocking of the pore region, which hinders the transport of potassium ions. This will increase the neuronal excitability and lower the threshold for generating an action potential. Other toxins that block SK channels are tamapin and scyllatoxin.
=== Genetic influence and environmental factors === Various studies have found direct genetic effects to account for at least 46% of the variation of timing of puberty in well-nourished populations. The genetic association of timing is strongest between mothers and daughters. The specific genes affecting timing are not yet known. Among the candidates is an androgen receptor gene. Researchers have hypothesized that early puberty onset may be caused by certain hair care products containing estrogen or placenta, and by certain chemicals, namely phthalates, which are used in many cosmetics, toys, and plastic food containers.
==== The riddle in popular culture ==== In Jean Cocteau's retelling of the Oedipus legend, The Infernal Machine, the Sphinx tells Oedipus the answer to the riddle in order to kill herself so that she did not have to kill any more, and also to make him love her. He leaves without ever thanking her for giving him the answer to the riddle. The scene ends when the Sphinx and Anubis ascend back to the heavens. There are mythic, anthropological, psychoanalytic and parodic interpretations of the Riddle of the Sphinx, and of Oedipus's answer to it. Sigmund Freud describes "the question of where babies come from" as a riddle of the Sphinx.
== Treatment == Surgery, with as wide a margin of removal as possible, has generally been the most effective and preferred way to attack LMS. If surgical margins are narrow or not clear of tumor, however, or in some situations where tumor cells were left behind, chemotherapy or radiation has been shown to give a clear survival benefit. While LMS tends to be resistant to radiation and chemotherapy, each case is different and results can vary widely. For metastatic (widespread) disease, chemotherapy and targeted therapies are the first choices. Chemotherapy regimens include doxorubicin/ifosfamide and doxorubicin combination/gemcitabine and docetaxel/trabectedin; pazopanib is the targeted therapy used in metastatic leiomyosarcoma as second line and is well tolerated. LMS of uterine origin often responds to hormonal treatments. As of 2020, several clinical trials for uterine LMS are active.

It is a dual GIP and GLP-1 receptor agonist, frequently grouped with incretin-based peptide therapeutics. It is a peptide rather than a small molecule and is given by subcutaneous injection.
Selective agents engage only the GLP-1 receptor, whereas tirzepatide activates GIP and GLP-1 receptors simultaneously. This difference in receptor coverage is the principal pharmacological distinction emphasised in comparative reviews.
Downstream signalling is partly characterised, but the quantitative contribution of GIP versus GLP-1 receptor activation to metabolic outcomes is not settled. Review articles commonly flag this as an unresolved question rather than a settled finding.
It acts as a dual agonist at the GIP receptor and the GLP-1 receptor. This broader targeting profile distinguishes it from selective GLP-1 agonists, which engage only one receptor.
