This is a working overview of tirzepatide, written for readers who want more than a one-paragraph summary but less than a textbook.

After subcutaneous injection, absorption is gradual, and peak plasma levels are generally reached within one to three days. Albumin binding extends the apparent half-life to roughly five days, which supports a weekly administration schedule. Metabolism proceeds mainly through proteolytic cleavage of the peptide backbone and beta-oxidation of the fatty acid chain, rather than through cytochrome P450 pathways. Eliminated fragments are largely recycled through general protein turnover, and excretion of intact drug in urine is minimal. These properties distinguish the molecule from short-acting incretin mimetics.
Tirzepatide is a synthetic peptide of 39 amino acids engineered from the native glucose-dependent insulinotropic polypeptide sequence. Its structure incorporates several non-natural residues and a C-terminal segment derived from glucagon-like peptide-1, together with a C20 fatty diacid moiety attached through a linker. The lipophilic side chain promotes binding to serum albumin, which slows renal clearance after administration. The compound is classified as a dual incretin receptor agonist and is supplied as a lyophilized powder for reconstitution or as a preformulated solution, depending on the presentation.
The peptide activates two G protein-coupled receptors, GIPR and GLP-1R. Binding triggers adenylyl cyclase activity and raises intracellular cyclic AMP in pancreatic beta cells, which potentiates insulin release when glucose is elevated. Signaling in the central nervous system is associated with reduced appetite and lower energy intake, while effects on gastric emptying and glucagon secretion are also reported. Because activity at both receptors is retained, the pharmacological profile is often described as incretin-based rather than selective for a single receptor.
Research and analytical settings increasingly require documentation of peptide origin and chain of custody. Certificate of analysis documents typically report purity by chromatographic area, mass confirmation, appearance, and residual solvent or counterion content. Independent verification by an accredited laboratory is common when a material will be used in a regulated study. Open questions remain about how well compendial methods transfer between laboratories, and about which impurity thresholds are meaningful for materials not intended for clinical use.
Peptide-based pharmaceutical products such as tirzepatide require controlled temperature management to preserve structural integrity. Manufacturer labeling generally specifies refrigeration at 2 to 8 degrees Celsius before first use, with protection from light and freezing. Exposure to repeated temperature cycling can promote aggregation or deamidation, which alters the analytical profile even when the visible solution appears unchanged. Once a product is in use, the permitted storage window and temperature range are defined by the specific labeled presentation rather than by general peptide rules.

Published work supports the view that engaging two incretin receptors produces changes in glucose handling and body weight larger than those seen with single-receptor activation. Why that difference arises is not fully settled. Open questions include how much of the observed weight effect depends on central versus peripheral signaling, and whether the two receptors form interacting complexes. Most reported findings come from controlled trials and animal models, and translation between species is imperfect. Further research is expected to refine these points over time.
Tirzepatide is a synthetic peptide built from 39 amino acid residues. Its sequence is related to human glucose-dependent insulinotropic polypeptide, with modifications that include a C-terminal extension and a C20 fatty diacid joined through a linker. Those changes raise the molecule's affinity for serum albumin, which slows renal filtration and lengthens the time it stays in circulation. The free base has an average molecular mass near 4813.5 daltons. The compound is made by solid-phase peptide synthesis followed by chromatographic purification.
At the receptor level, tirzepatide activates both the glucose-dependent insulinotropic polypeptide receptor and the glucagon-like peptide-1 receptor. Both belong to the class B family of G protein-coupled receptors and signal largely through cyclic AMP accumulation. The compound binds the two receptors with differing affinity, and the pattern of signaling at each site is described in the literature as biased rather than simply proportional to occupancy. Tissues carrying these receptors include pancreatic islets, adipose tissue, the central nervous system, and the gastrointestinal tract. The relative weight of each receptor population in producing metabolic effects continues to be studied.
==== Hovertrain ==== Early in 1973, rumours began to circulate that the Tracked Hovercraft (known as the "Hovertrain"), a planned 300 mph floating train on which work had begun in 1967, was to be cancelled. On 12 February 1973, Heseltine gave a written answer on Peter Walker's behalf to a written question from Labour MP David Stoddart, that a further injection of government money was still "under consideration". However, two days later Heseltine appeared before the Select Committee, and revealed that the government had already decided to pull the plug on the Hovertrain on 29 January. Airey Neave believed Heseltine had been lying and urged Stoddart to pursue the matter. The Hovertrain incident came to be regarded as the worst example of lying to the House of Commons since the Profumo affair a decade earlier, and Heseltine survived because full details only emerged during the Parliamentary summer recess. The committee's report in September accused Heseltine of having given an "untrue" answer on 12 February. Heseltine immediately gave a press conference (7 September 1973) in which he denied that he had lied. On the orders of Chief Whip Francis Pym he apologised to the House of Commons on 16 October 1973 for having made a statement which was open to "more than one interpretation".

Amidorphin is an endogenous, C-terminally amidated, opioid peptide generated as a cleavage product of proenkephalin A in some mammalian species; in humans and most other species, the peptide is 1 residue longer and is not amidated. Amidorphin is widely distributed in the mammalian brain, with particularly high concentrations found in the striatum, and outside of the brain in adrenal medulla and posterior pituitary. The 26-residue peptide named amidorphin is found in several species including bovine (Bos taurus), sheep (Ovis aries), and pig (Sus scrofa). Humans and commonly studied lab animals (mice, rats) produce a 27-residue peptide that does not have an amidated C-terminal residue; this is due to the absence of a Gly in the precursor sequence and replacement with Ala, which is not a substrate for the amidating enzyme (Peptidyl-glycine alpha-amidating monooxygenase). The properties of the 27-residue peptide are presumably similar to those of amidorphin, although this has not been adequately tested. In some brain areas, amidorphin is extensively further reduced into smaller fragments, such as the non-opioid peptide amidorphin-(8-26), or in humans, amidorphin-8-27. Cleavage of amidorphin into these smaller fragments releases the N-terminal [Met]-enkephalin sequence of amidorphin.
== Chemistry == Expired tetracyclines or tetracyclines allowed to stand at a pH less than 2 are reported to be nephrotoxic due to the formation of a degradation product, anhydro-4-epitetracycline causing Fanconi syndrome. In the case of doxycycline, the absence of a hydroxyl group in C-6 prevents the formation of the nephrotoxic compound. Nevertheless, tetracyclines and doxycycline itself have to be taken with caution in patients with kidney injury, as they can worsen azotemia due to catabolic effects.
Unlike women, men do not experience reliable indicators of elevated prolactin such as menstrual changes, to prompt immediate medical consultation. As a result, the early signs of hyperprolactinemia are generally more difficult to detect and may go unnoticed until more severe symptoms are present. For instance, symptoms such as loss of libido and sexual dysfunction are subtle, arise gradually, and may falsely indicate a different cause. Many men with pituitary tumor–associated hyperprolactinemia may forego clinical help until they begin to experience serious endocrine and vision complications, such as major headaches or eye problems. Men often present late in the course of hyperprolactinemia, typically with symptoms related to the expansion of their pituitary tumor, such as headaches, visual defects, and external ophthalmoplegia, or symptoms from secondary adrenal or thyroid failure. Despite experiencing sexual impairment for many years before receiving a diagnosis, it is unclear whether macroprolactinomas are more commonly seen in men due to delayed diagnosis or if the pathogenesis of prolactinomas differs between men and women. Unlike women, who most commonly have microprolactinomas, men usually present with macroprolactinomas, and their serum prolactin levels are generally much higher than those observed in women. Long-term hyperprolactinaemia can lead to detrimental changes in bone metabolism as a result of hypoestrogenism and hypoandrogenism.

Follow-up longitudinal studies of women with prosthetic breasts indicated no causal relation between the presence of a breast prosthesis and the occurrence of either a systemic disease or an autoimmune disease. European and North American studies reported that women who had undergone an augmentation mammoplasty tended to be healthier than the general population; that plastic surgery patients had a lower standardized mortality ratio than did other types of surgery patient; and that women with prosthetic breasts faced a greater rate of incidence for lung cancer than did other types of plastic-surgery patient. Moreover, because only the study Long-term Cancer Risk among Swedish Women with Cosmetic Breast Implants: an Update of a Nationwide Study (2006) controlled for tobacco smoking, the data were insufficient to establish verifiable statistical differences between smokers and non-smokers and the greater incidence of death by lung cancer for women with prosthetic breasts. The long-term study of 25,000 women, Mortality among Canadian Women with Cosmetic Breast Implants (2006), reported that breast prostheses do not directly increase mortality in women. The study Silicone-gel Breast Implant Rupture, Extracapsular Silicone, and Health Status in a Population of Women (2001) reported an increased rate of incidence of fibromyalgia among women who had suffered a capsular contracture that leaked silicone-gel, than among women whose prosthetic breasts had neither ruptured nor leaked.
== Epidemiology == Pooled data suggest the incidence of NMS is between 0.2%–3.23%. However, greater awareness coupled with increased use of atypical anti-psychotics have likely reduced the prevalence of NMS. Additionally, young males are particularly susceptible and the male to female ratio has been reported to be as high as 2:1.
== Manufacture == Portland cement is made by heating limestone (calcium carbonate) with other materials (such as clay) to 1,450 °C (2,640 °F) in a kiln, in a process known as calcination that liberates a molecule of carbon dioxide from the calcium carbonate to form calcium oxide, or quicklime, which then chemically combines with the other materials in the mix to form calcium silicates and other cementitious compounds. The resulting hard substance, called 'clinker', is then ground with a small amount of gypsum (CaSO4·2H2O) into a powder to make ordinary Portland cement, the most commonly used type of cement (often referred to as OPC). Portland cement is a basic ingredient of concrete, mortar, and most non-specialty grout.
Diagnosis of ILD involves assessing the signs and symptoms as well as a detailed history investigating occupational exposures. ILD usually presents with dyspnea, worsening exercise tolerance and 30-50% of those with ILD have a chronic cough. On examination, velcro crackles, in which the crackles compare to the sound of velcro being unfastened, are common in ILD. Pulmonary function tests usually show a restrictive defect with decreased diffusion capacity of carbon monoxide (DLCO) indicating reduced alveolar to blood capillary transport. Pulmonary function testing is indicated for all people with ILD and the FVC loss and DLCO is prognostic, with an FVC loss of greater than 5% per year associated with a poor prognosis in fibrosis subtypes of ILD. A chest x-ray is 63% sensitive and 93% specific for ILD. With advances in computed tomography, CT scans of the chest have supplanted lung biopsy as the preferred diagnostic test for ILD. A thoracic CT scan is 91% sensitive and 71% specific for ILD. In higher income countries, less than 10% of people with ILD undergo a lung biopsy as part of the diagnostic evaluation. A lung biopsy may be required if the clinical history and imaging are not clearly suggestive of a specific diagnosis or malignancy cannot otherwise be ruled out. Surgical lung biopsy or via a video-assisted thoracoscopic surgery (VATS) biopsy is associated with a mortality rate up to 1-2%.

Suzetrigine, sold under the brand name Journavx, is an analgesic medication used in the treatment of moderate to severe acute pain. It is taken orally. Suzetrigine was developed by Vertex Pharmaceuticals. The most common adverse effects include itching, muscle spasms, increased blood level of creatine phosphokinase, and rash. Suzetrigine is a small-molecule non-opioid analgesic that works as a selective inhibitor of Nav1.8-dependent pain-signaling pathways in the peripheral nervous system. Nav1.8 channels are predominantly present in peripheral nociceptive neurons of the dorsal root ganglia. Suzetrigine inhibits pain signals before they reach the central nervous system and has no addictive potential. Suzetrigine was approved for medical use in the United States in January 2025. The US Food and Drug Administration considers it to be a first-in-class medication.
==== E-I balance as crucial for visual cortex function ==== By computational modeling, Sur’s group proposed that a key feature of visual cortex networks - local and long-range connections between excitatory neurons critically balanced by inhibition - is essential for explaining feature-selective responses. They demonstrated that inhibition and excitation are exquisitely tuned across cortical sites, as demonstrated by imaging and intracellular recordings.
Systemic Lupus Erythematosus (SLE) Systemic Sclerosis (SS) Primary Sjögren's Syndrome (pSS) Raynaud's phenomenon Scleroderma Rheumatoid Arthritis The addition of anti-U1 RNP antibodies reveals that many autoimmune diseases share symptoms such as joint pain, fatigue, and Raynaud's phenomenon. The pathogenic role of anti-nRNP antibodies in these diseases varies, and their presence may only reflect immune system deregulation rather than being the direct cause of disease symptoms. Yet, they have served as valuable biomarkers.
== Adverse effects == The most common adverse effects of suzetrigine may include itching, rash, muscle spasms, and increased levels of creatine kinase. Mild side effects may include nausea, vomiting, constipation, headache, and dizziness. In preliminary research, suzetrigine had no serious neurological, behavioral, addictive or cardiovascular effects. As of 2024, long-term safety in broader contexts including multimodal analgesia, pregnancy and breastfeeding women remain undetermined.

It is a synthetic linear peptide that acts as a dual agonist at the GIP and GLP-1 receptors. It combines a modified incretin backbone with a fatty diacid side chain that extends its circulation time. It is not a small-molecule drug and is not orally absorbed in its native form.
Single-receptor agents act predominantly at GLP-1R. Tirzepatide retains activity at GIPR, which is expressed in pancreatic islets, adipose tissue, and the central nervous system. The contribution of each receptor to the overall response is still an area of active study, and the relative weight of GIPR signaling in humans is not fully settled.
The fatty diacid moiety binds serum albumin, which reduces renal filtration of the active peptide. Additional resistance to enzymatic breakdown comes from non-natural residues in the backbone. Together these features produce an apparent half-life of about five days and permit weekly dosing intervals.
The peptide backbone and its fatty acid side chain are susceptible to degradation at elevated temperatures. Refrigeration slows hydrolysis, oxidation, and aggregation processes. Labeled storage ranges reflect stability data generated under defined conditions.
